Medalogix Announces DeVero EHR Compatibility
A large healthcare system is rolling outBridge—an analytics technology that predicts the probability of death to facilitate better conversations about end-of-life care options—across all its home health, dual operator branches. To accommodate the enterprise-wide expansion, Medalogix developed an integration with the provider’sElectronic Health Record (EHR), DeVero.
Bridgeanalyzes home health patient data to identify and rank patients at risk of passing away within 90 days. Then, the solution facilitates action and communication between service lines with its workflow component. Once a patient’s clinical evidence is reviewed, clinicians can move each patient through organization-chosen workflow steps, such as,Complete Clinical EvidenceandObtain Physician Orderto better manage and coordinate care leading toward appropriate end-of-life care conversations. The steps are timed to help pinpoint operational lag.
Earlier and more accurate hospice-appropriate identification has the potential to drastically positively affect the healthcare system as a whole. According to the National Hospice and Palliative Care Organization, of the more than 1.5 million patients who receivedhospicecare in the United States in 2013, one-third died within one week of getting it—so patients who want hospice care, are learning about it too late. Not only is that detrimental to care quality, but it creates financial waste. Twenty-eight percent of Medicare dollars—about $170 billion annually—are spent during a patient’s last six months of life, often on futile treatments that lead to suffering.
