The Home Health Patient Journey with Medalogix's Population Health Analytics

When analytics are leveraged at every step of a home health patient’s journey, outcomes improve and big picture costs for patients and providers decrease.

While we often talk about our products separately according to what they each help facilitate (readmission reduction, timely hospice care conversations, additional care needs and evidence-based care plans), we wanted to zoom out and take a look at the holistic view of our product suite. To do that, we’ve created the infographic below. 

If you’re not familiar with the typicalhome health journey, it’s important to note that it’s centered around a 60-day patient care episode. That means when patients are admitted to home health, their treatment is plotted on a 60-day timeline. After that 60 days, they can remain at home without care, readmit to another 60-day home health episode or sometimes, if their condition worsens, go back to a care facility like skilled nursing or the hospital. With all of this in mind, you’ll see the timeline on the left side of our infographic.

You’ll also see our solutions are divided by their relevanceto low or high-risk patients. Typically, your high-risk patients will benefit most from analytics-based monitoring. If they’re likely to return to the hospital, benefit from hospice care or benefit from an additional care episode, they’re relatively riskier patients who will require timely and appropriate interventions. Medalogix will ensure you know who these patients are and have the tools to effectively and efficiently intervene to achieve the optimal care quality.

Patients who are lower risk can benefit from our new prescriptive analytics solution, Path, which helps prescribe evidenced-based care disciplines and frequencies. For example, based on millions of patient records, Path can generate the most statistically effective care plan for a home health patient. It can inform a provider that patient Joe Smith will experience the best outcome with 3 physical therapy visits, 2 times a week for 4 weeks. While we can also prescribe this for high-risk patients, those patients may have to experience other lines of care, like hospice, or an additional episode of care to achieve the best outcome.
