Medalogix helps home health provider to identify those ready for hospice care discussion
“Medalogix came to us with a predictive analytics technology to help us identify the most appropriate levels of care for our patients,” said Peter Egan of EGAN Home Health & Hospice. “It will benefit our healthcare delivery system by identifying at an early stage those who may be at risk for health decline and their need to go to the hospital or emergency room. It allows us to put in place interventions to change this.”
Bridge, the technology Egan is using, identifies patients who could benefit from hospice care and then equips providers with a way to work that knowledge into their regular online paperwork and clinical notes. The solution is especially useful for dual home health and hospice providers, adding predictive insights and operational efficiency to their hospice transition process.
EGAN needs this kind of information to comply with federally mandated quality requirements governing hospice provider communications with patients and families.
The Bridge solution was recently recognized by Harvard University as an impactful healthcare innovation when Medalogix was selected from a pool of 500 applicants as a finalist in its Health Acceleration Challenge.
